SUMMARY DESCRIPTION OF WORK

Certified Community Behavioral Health Clinic (CCBHC) helps provide comprehensive, coordinated mental health and substance use services to the individual. CCBHC utilizes an integrated person, and family centered treatment plan to help ensure the individuals needs are being met. CCBHC offers access to care for any individual seeking help for mental health, substance use, or community support regardless of diagnosis, residence, or ability to pay. The goal of CCBHC is to provide a one stop shop for behavioral health services to the individual.

Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience)
Work Schedule: 37.5 hours per week

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in psychology, social services, sociology, or other related fields
  • Experience working with individuals at risk of suicide, and chronic substance use
  • Experience working with individuals at risk of housing instability and/or unhoused.
  • Five years’ experience in health and human services, or related field

Essential Duties and Responsibilities

  • Provide access point for CCBHC client enrollments.
  • Document client contact, Release of Information, consents for treatment, and any additional documentation that may be needed.
  • Provide high intensity case management services for individuals with complex needs who are at risk of losing stable housing.
  • Engage these individuals in appropriate behavioral programming to address underlying symptoms and behaviors effecting housing stability.
  • Establish rapport with landlords to communicate in collaboration with clients.
  • Demonstrates understanding of culture of homelessness and poverty as well as understanding of substance use disorders and severe and persistent mental illness and associated symptoms.
  •  Refer to appropriate resources/services, which meet the client’s needs to include physical health, behavioral health/substance use, social services, housing, education, and employment. 
  • Interface effectively and advocate for clients with other HFS Housing Services and other HFS programs, Iowa Legal Aid, Municipal Housing Authority, hospitals, landlords, and other community stakeholders and systems.
  • Help engage and motivate clients into treatment/programming by utilizing a motivational interviewing skillset.
  • Work with individuals in times of transition to include transition of treatment and individuals who may be at high risk of suicide, psychiatric hospitalization, individuals at higher risk of overdose, homelessness, emergency department stays, and release from jail/prison.
  • Works with individuals’ short term to resolve behavioral health concerns that cause housing instability and refers to client’s care team when they are stabilized and engaged in appropriate programming.
  • May provide transportation to individuals.
  • *Create, maintain and share as appropriate a dynamic self-care plan.
  • * Strive to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • * Essential functions of this job are to be performed on company physical work site or designated workspace.
